Archived Support / Fatal Exception Reseting D3D Device Failed?

I keep getting this (Fatal Exception: Reseting D3D Device Failed) and the map does not appear.  Any ideas?  There are also segments of the map that while say they are loading, do not.  Thanks for any help!
